At PA Health Management we provide assistance to help you navigate and check on the development of your application with the Independent Enrollment Broker. It has been shown that the process can move quicker when applicants choose the assistance of an advocate. Each waiver offered by OLTL is different in reference to the population and the number of people it can help, the criteria in order to qualify, and what services are provided. To apply for waiver services, applicants must go through the enrollment and qualification process which will be overseen by Pennsylvania’s Independent Enrollment Broker. Be assured that through every step of the process our team will be working hard to help you receive every benefit possible.
There are many Home and Community Based Waiver Programs that are available to you through Office of Long Term Living (OLTL) and Bureau of Autism Services (BAS). We are an approved provider for the following programs:
Individuals aged 18-59 with physical disabilities who are mentally alert and can control their own services. For attendant care waiver individuals must be at a nursing facility level of care.
Individuals aged 21 and older with a medically diagnosed traumatic brain injury. This waiver helps to maintain independence while preventing institutionalization.
Individuals aged 18-60 with permanent physical disabilities that have three or more limitations such as mobility, communication, self-care, self-direction, independent living, or learning.
Individuals with developmental physical disabilities who require facility level of care but are striving to maintain as much independence as possible.
Individuals aged 21 and older diagnosed with Autism Spectrum Disorder which helps to maintain independence while preventing institutionalization.
Individuals aged 60 and older who meet a nursing facility level of care. This waiver helps individuals maintain independence in the community of their choice.
